NumberGuru is a reverse phone lookup, so its listings are keyed to numbers rather than names. Its opt-out redirects to BeenVerified, which processes the request. Phone-keyed listings are the ones people miss, because a search on your own name may return nothing while a search on your number returns you.
NumberGuru's opt-out requests are processed on BeenVerified's system, so one request can cover more than one site. Check both afterwards rather than assuming.

Step-by-step removal

At the bottom of numberguru.com click Do Not Sell My Personal Information.

Continue to BeenVerified
Click the green Do Not Sell My Info button on the notice that appears.

Search for your record
Enter your first name, last name and state, then click Search.

Select your profile
Find your entry and click Proceed to Opt Out.

Submit an email
Enter an email address, clear the CAPTCHA and click Send Verification Email.

Verify
Open the message from BeenVerified and click Verify Opt-Out.
