In the People Search area of veripages.com enter your first and last name plus city and state, then run the search.

Locate your listing and click View Profile at the top right.

Scroll to the bottom of the profile and click Control Profile, which opens the opt-out page.

The Profile URL field is filled in for you. Add your full name and an email address, clear the CAPTCHA and click Submit Opt Out Request.
